Welcome to your Patient Portal.

If you have not registered to access this Portal please contact one of our three offices and have them add your email address to your patient information and you will be sent a ‘token’ – an instruction sheet with your private log-in number. The Portal provides you private electronic access to your medical records, ask questions about your treatments and exams, request medications and other information.

* The Patient Portal does not replace appointments, examinations or testing. Doctors will not diagnose problems, order treatments and will not develop plans of care based on questions or problems submitted through the Portal. You must be seen in the office. Thank you.
